Type and print out the following:
 • One-page Medical History
 • List of symptoms and concerns

Step 4

Bring completed forms and documents (Steps 2 and 3) as well as your most recent lab work (including Lyme testing) with you to your appointment.

Telemedicine

Phone consultations are available to long-distance patients out of state or overseas. An office visit may be required during diagnosis and treatment. Fees are billed per length of the call. Contact our office for the latest prices and to make or change an appointment.

Billing Policy

Full payment for all fees are due at the time of each appointment (either phone calls, in person, or at IV services). We accept most major credit cards.

We do not bill insurance.

We do not accept Medicare and are not Medicare providers.


Cancellation Policy

If you wish to cancel an appointment we require a minimum notice of 48 hours or you will be charged 100% of the appointment fee.
